To factory reset a Polycom VVX 150, power off the phone and then power it back on while holding down the "1," "3," and "5" keys simultaneously. Keep these keys pressed until the "Enter Password" prompt appears, then enter the MAC address of the phone as the password to initiate the reset. This will restore the phone to its original factory settings.
